Strep throat symptoms and treatment

What is Streptococcus?

Streptococcus is a bacterial infection caused by the bacteria Streptococcus pyogenes, also known as group A streptococcus. This infection leads to inflammation and pain in the throat, making it feel sore and scratchy. Strep throat is a common illness, particularly in children, but it can affect people of all ages. While most cases of sore throat are viral, strep throat accounts for a small portion of sore throats and requires antibiotic treatment to prevent complications.

Prompt treatment of strep throat is important because, if left untreated, it can lead to serious health issues such as kidney inflammation or rheumatic fever. Rheumatic fever can cause permanent damage to the heart valves, inflammation of the joints, and other complications. Fortunately, early diagnosis and treatment with antibiotics can help prevent these outcomes.

Symptoms of Strep Throat

The symptoms of strep throat can appear suddenly and vary depending on the severity of the infection. Common signs and symptoms include:

  • Throat pain that often comes on quickly.
  • Painful swallowing that makes eating or drinking difficult.
  • Red and swollen tonsils, sometimes with white patches or streaks of pus.
  • Tiny red spots on the area at the back of the roof of the mouth (soft or hard palate).
  • Swollen and tender lymph nodes in the neck.
  • Fever, often over 101°F (38.3°C).
  • Headache.
  • Rash (sometimes called scarlet fever).
  • Nausea or vomiting, especially in young children.
  • Body aches.

In some cases, people may have mild symptoms or carry the bacteria without showing any signs of illness. These individuals are known as asymptomatic carriers and can still spread the bacteria to others without being sick themselves.

When to See a Doctor

Because many of the symptoms of strep throat can overlap with other illnesses, such as viral infections, it’s important to consult a doctor for proper diagnosis. A test is needed to confirm whether you or your child has strep throat. Contact your healthcare provider if you or your child has any of the following symptoms:

  • A sore throat accompanied by swollen, tender lymph glands.
  • A sore throat that lasts longer than 48 hours.
  • A fever, especially if it is persistent or high.
  • A sore throat accompanied by a rash.
  • Difficulty breathing or swallowing.
  • No improvement after 48 hours of taking antibiotics for confirmed strep throat.

Complications of Strep Throat

If left untreated, strep throat can lead to serious health complications, including:

Rheumatic Fever

Rheumatic fever is a serious inflammatory disease that can develop after a strep throat infection. It can affect the heart, joints, skin, and brain, and may cause permanent heart damage known as rheumatic heart disease. Symptoms of rheumatic fever include:

  • Painful and inflamed joints, particularly in the knees and ankles.
  • A specific type of rash, often with red patches.
  • Fever.
  • Fatigue and shortness of breath.

Kidney Inflammation (Post-Streptococcal Glomerulonephritis)

Post-streptococcal glomerulonephritis is an inflammation of the kidneys that can occur after a strep throat infection. This condition can cause:

  • Swelling in the face, hands, and feet.
  • Blood in the urine (which may look pink or brown).
  • High blood pressure.

Diagnosis of Strep Throat

To determine if you or your child has strep throat, your doctor will perform a physical examination and may use one or both of the following tests:

Rapid Antigen Test

A rapid antigen test involves swabbing the throat to detect the presence of group A streptococcus bacteria. Results are usually available within minutes. If the test is positive, your doctor may start treatment immediately.

Throat Culture

If the rapid test is negative but strep throat is still suspected, a throat culture may be done. A sample from the throat is cultured in a lab to determine if strep bacteria grow. This test is more accurate but takes 1-2 days for results.

Treatment of Strep Throat

Antibiotics are the standard treatment for strep throat. They help reduce symptoms, prevent complications, and decrease the likelihood of spreading the infection to others. The most commonly prescribed antibiotics include:

  • Penicillin or amoxicillin.
  • For people allergic to penicillin, alternative antibiotics such as azithromycin or clindamycin may be prescribed.

It’s important to complete the entire course of antibiotics, even if symptoms improve after a few days, to ensure the bacteria are fully eliminated and prevent recurrence or complications.

Home Care and Symptom Relief

In addition to antibiotics, the following home care measures can help relieve symptoms:

  • Rest to help your body fight the infection.
  • Stay hydrated by drinking plenty of fluids, such as water or warm teas.
  • Take pain relievers like ibuprofen or acetaminophen to reduce fever and relieve throat pain.
  • Gargle with warm salt water to soothe the throat (for older children and adults).
  • Use throat lozenges or hard candy to relieve throat discomfort.

Preventing Strep Throat

While it’s not always possible to prevent strep throat, there are steps you can take to reduce the risk of infection:

  • Wash your hands frequently, especially after coughing or sneezing.
  • Avoid sharing eating utensils, drinks, or personal items with others.
  • Cover your mouth and nose when coughing or sneezing to prevent spreading germs.
  • If someone in your household is diagnosed with strep throat, disinfect surfaces they may have touched, including doorknobs and countertops.

Conclusion: Early Treatment is Key

Strep throat is a common bacterial infection that can lead to serious complications if left untreated. Early diagnosis and treatment with antibiotics can prevent these complications and reduce the severity of symptoms. If you or your child has symptoms of strep throat, such as a sore throat, fever, or swollen lymph nodes, contact your doctor for testing and appropriate treatment. Remember, taking simple preventive measures such as washing your hands regularly can help lower the risk of infection.
